The Court Supervised Settlement Program commenced as of June 4, 2012. Claim Forms are available on this website and in Claimant Assistance Centers, and claimants are now able to submit Claim Forms via mail or online. Depending on the claim type(s) relevant to each claimant, these Claim Forms require supporting documentation in the form of Sworn Written Statements and/or Authorizations. Claimants should consult the Instruction Booklet(s) relevant to their claim type(s), as well as their Claim Form(s) for guidance on which Sworn Written Statements and/or Authorizations apply to them.
